Hello, everyone. My name is Andy and I am a 43 year old married male from Alabama. I was born and raised in Alabama. I graduated from Brewer High School in 1984. I served in the U.S. Army for 4 years (1985 - 1989) with 3 years of my time in service in Grafenwohr, Germany. I have been employed by CDI Electronics in Madison for the last 20 plus years. For 19 plus of those years, I was the Purchasing Manager. In January 2009, I was moved to Docmentation and IT to assist in building our MRP database due to my many years of knowledge and experience with the company. >

In 1995, I married my wife Sherry, who is a nurse. We have been married for over 13 wonderful years. I am a proud Father and Dad of two children, a 11 year old daughter and 8 year old son.

We attend Somerville Baptist Church. Our Pastor, Bro. Shane Lewis is a great preacher. Our Somerville Church family is truly an extended part of our family. We love our church family.

In December 2006, I became a neck-breather as I had to get a tracheotomy for my obstructive sleep apnea. Obstructive sleep apnea (OSA) is the most common category of sleep-disordered breathing. The muscle tone of the body ordinarily relaxes during sleep and at the level of the throat the human airway is composed of collapsible walls of soft tissue which can obstruct breathing during sleep. Individuals with low muscle tone and soft tissue around the airway (e.g., due to obesity), and structural features that give rise to a narrowed airway are at high risk for obstructive sleep apnea. Men are more typical sleep apnea sufferers than women and children, although it is not uncommon in the latter two. OSA can kill. Snoring is almost a uniform finding in an individual with this syndrome. Although not everyone who snores is experiencing difficulty breathing, "snoring" in combination with other conditions such as overweight and obesity has been found to be highly predictive of OSA risk. The loudness of the snoring is not indicative of the severity of obstruction, however. If the upper airways are tremendously obstructed, there may not be enough air movement to make much sound. Even the loudest snoring does not mean that an individual has sleep apnea syndrome. The sign that is most suggestive of sleep apneas occurs if snoring stops. If it does, along with breath, while the persons' chest and body tries to breathe - that is literally a description of an event in obstructive sleep apnea syndrome. When breathing starts again, there is typically a deep gasp, and then the resumption of snoring. A sleep study must be performed to determine if you have sleep apnea and to determine the course of treatment. The most common treatment and arguably the most consistently effective treatment for sleep apnea is the use of a continuous positive airway pressure (CPAP) device, which 'splints' the patient's airway open during sleep by means of a flow of pressurized air into the throat. However the CPAP machine only assists inhaling whereas a BiPAP machine assists with both inhaling and exhaling, and is used in more severe cases.

In addition to CPAP, a dentist specializing in sleep disorders can prescribe Oral Appliance Therapy (OAT). The oral appliance is a custom made mouthpiece that shifts the lower jaw forward which opens up the airway. OAT is usually successful in patients with mild to moderate obstructive sleep apnea. OAT is a relatively new treatment option for sleep apnea in the United States, but it is much more common in Canada and Europe. CPAP and OAT are effective only for obstructive sleep apnea, not for central or mixed cases. In mild cases of obstructive sleep apnea, use of a specially shaped pillow or shirt may reduce sleep apnea episodes, usually by causing users to sleep on the side instead of on the back or in a reclining position instead of flat.

For patients who do not tolerate or fail non-surgical measures, surgical treatment to anatomically alter the airway is available. Several levels of obstruction may be addressed, including the nasal passage, throat (pharynx), base of tongue, and facial skeleton. Surgical treatment for obstructive sleep apnea needs to be individualized in order to address all anatomical areas of obstruction. Often correction of the nasal passages needs to be performed in addition to correction of the oropharynx passage. Septoplasty and turbinate surgery may improve the nasal airway. Tonsillectomy and uvulopalatopharyngoplasty (UPPP or UP3) is available to address pharyngeal obstruction. Base of tongue advancement by means of advancing the genial tubercle of the mandible may help with the lower pharynx. A myriad of other techniques are available, including hyoid bone myotomy and suspension and various radiofrequency technologies. For patients who fail these surgeries, the facial skeletal may be advanced by means of a technique called maxillo-mandibular advancement, or two-jaw surgery (upper and lower jaws). Technically, this is accomplished by a surgery similar to orthognathic surgeries addressing an abnormal bite. The surgery involves a Lefort type one osteotomy and bilateral sagittal split mandibular osteotomies. Final option is a tracheotomy.

I tried the CPAP, BiPaP, with and without oxygen. I had uvulopalatopharyngoplasty (UPPP or UP3) surgery along with septoplasty surgery in 2001. It was determined that maxillo-mandibular advancement would not help me as much as a tracheotomy.

In January 2008, I was approved for the Lap-Band to help with my weight and other health problems. A laparoscopic adjustable gastric band (Lap-Band) is a restrictive device implanted via bariatric surgery and designed for obese patients with a body mass index (BMI) of 40 or greater—or between 35–40 in cases of patients with certain comorbidities that are known to improve with weight loss, such as sleep apnea, diabetes, osteoarthritis, GERD, or metabolic syndrome, among others. The gastric band is an inflatable silicone device that is placed around the top portion of the stomach, usually via laparoscopic surgery. In February 2008, I was banded. I have lost over 45 lbs to date. I have managed to lose the need for 3 of my 4 mediciations in the last year. I have two goals to reach. 1st goal is to get under 200 lbs to have a capped trach sleep study performed to evaulate the prospect of removing my trach. I hope to reach this goal by the later part of 2009. 2nd goal is to meet my goal weight of 165-170 lbs. I have about 45 lbs to go to reach my 1st goal and 80 lbs to reach my second goal. The process is slow but steady at a rate of 1-2 lbs per week. Tho' some weeks I don't lose any weight. I do have NSV which is non-scale victories. An example of an NSV is dropping a pants size, which I have completed since the begining of 2009. My waist size was a 44 in February 2008. I currently can wear a size 38. By goal, I hope to be in a 34 waist size pant, if not a 32.
